Which doctors specialize in treating male infertility problems?

Urologists or Genitourinary Surgeons are ideally suited to treat male infertility problems by virtue of their training and experience in male sexual organs. However, general urologists do not have adequate experience in Microsurgery and many of them have no exposure to Advanced Assisted Reproductive Technologies like ICSI.

It is necessary that you identify specialty centers where there is total care for the treatment of Infertility; Infertility is a field where many couple on misinformation waste their presious time, perform unnecessary laboratory tests and undergo surgical procedures.

What is Semen, How do I collect ? What tests are performed ?

Semen is the fluid that a man ejaculates. The sperms are necessary to fertilize the egg. Hence it is important to assess the quality of sperms to assess the fertility in the male. The semen on ejaculation is more viscid. It gets liquefied in about 10 minutes.

    a. Collection of Semen sample - Instructions

  • Abstain from sex for a period of atleast 3 to 4 days and not more than 7 days.
  • Produce a sample by masturbation only.
  • Collect the whole sample in a container provided.
  • The sample should reach laboratory in less than an hour. Ideally the specimen should be collected in the laboratory itself.

  • Semen Analysis when improperly or incompletely performed may miss significant problems. Unlike many other tests semen analysis relies completely on the expertise of those performing it. Ideally the laboratory should have facilities to do certain additional tests on the same semen sample.

How often should I have intercourse?

The frequency of intercourse depends solely on the desire of the couple; Intercourse every other day keeps the sperm reach the egg during ovulation periods. The sperms survive in female reproductive tract for 48 hours.

Does it matter what position we use during intercourse? Does leaking of semen out of vagina affect fertility?

Positions during intercourse should not adversely affect fertility and subsequent pregnancy. Loss of some semen from vagina is experienced by most women following intercourse and does not indicate and does not indicate that enough semen is leaking out to prevent pregnancy. If fertility is a problem, minimize semen loss and maximize contact of the center with the seminal pool in the vagina. This is achieved by women lying on her side with knees brought up towards her chest following sexual intercourse. Passing urine prior to intercourse avoids visits to the toilet soon after intercourse.

Can I use Lubricants during intercourse?

Most vaginal lubricants including K.Y.Jelly are toxic to the sperm. Better avoid using them during the fertile periods.

Does Masturbation affect fertility?

Masturbation is not different from normal Intercourse in that an orgasm causes ejaculation whether it is the result of masturbation or sexual relations. Avoid masturbating during wife's fertility period since masturbation will further lower the sperm quality.

What are the lifestyle changes that I should adopt to get my wife pregnant?

  • Avoid excessive heat (hot tubs)
  • Do not smoke
  • Exercise regularly and moderately
  • Have good nutritional habits - diet rich in fruits and leafy vegetables
  • Seek emotional and/or psychological support. Consider stress relieving measures

How does smoking affect fertility ?

Smoking cause decrease in sperm density and motility and increase in number of abnormal forms. Smoking affects Hypothalamo - Pituitory - Gonodotrophic axis affecting the levels of estrodial.

Which are the commonly used drugs that affects sperm production or function ?

  • Antibiotics - Furadantin, Gentamycin
  • Cimetidine (Drugs used for peptic ulcer)
  • Anti Psychotic Drugs - Largactil, Haloperidol
  • Anti Depressents - Imipramine, Amitriptyline
  • Anti Hypertensive - Resepine, Thiazides
  • Anabolic Steroids (Male Hormones)
